Assessment Tools - The Sharing Site
The following evaluation forms, surveys, rubrics, etc. have been presented by Lake Land College faculty for sharing.

Field Experience Evaluation Forms: A number of faculty use SOE (supervised occupational expeience), internship, or clinical evaluations as a part of their program assessment. 

Client/Patient Evaluation Forms:  A number of Lake Land College programs have labs or clinics in which clients or patients are served by students.  The following surveys are used to obtain information directly from clients/patients served by Lake Land students.

Graduate Surveys:  The following surveys are used by faculty to obtain feedback from program graduates. 

Rubrics:  The following rubrics are used to measure student performance in various areas.

Illinois Articulation Initiative (IAI) information -- http://www.iTransfer.org/-- View the IAI requirements to help guide goals for assessing general education and transfer programs.

1) Click iTransfer Gen. Ed., click any of the Gen. Ed. Core Requirements links (Communications, Mathematics, Physical Sciences, etc.), and click the definitions link to find definitions of each general education requirements, which can be used to create competencies.

2) From the iTransfer home page, click iTransfer Gen. Ed. and the Majors tab, scroll down to the "Majors that are currently available" drop-down menu, find your program major, select it and click submit. IAI has provided major descriptions from which goals can be pulled.
